Athletics: Student Activities Fees for Middle Level Sports

• Coaches will distribute the Student Activities Fee form (PDF) to athletes at the first meeting (or practice) for parents to fill out. The form and $150.00 payment is then returned to the coach. If the fee is not paid by the first official game, the athlete cannot participate. If an athlete quits the sport, it must be before the first official game in order to get a refund.

• A Student Activities Fee form (PDF) must be completed for every sport. Parents should check box on the form if the fee was already paid.
